Please bring your insurance card(s) with you to the appointment. If you have any questions, feel free to call us at 760-942-2524 ext. 21. Please plan to arrive 30 minutes before your scheduled appointment. If you are unable to keep your scheduled appointment, please notify our office at least 24-hours prior. If you are taking any medications, please bring a list of the names, dosage and frequency of these medications or you may bring the bottles. To eliminate wait times, please complete all forms prior to your first visit; if they are not complete upon arrival, your appointment may be delayed and/or rescheduled.
